Media Release Forms

Background

If a project contains media that features a person under 18 years old, then the minor must sign a Media Release Form in order for SARE to use the media.

On the project page, you will need to upload any signed Media Release Forms.

Getting There

1. On the home page of https://projects.sare.org/, click "Search Projects" in the upper right corner in the menu. 
2. Enter the project name or project number, and then click the "Search" button.
3. In the list of results, click the name of the project you are looking for. 

Upload a Media Release Form

1. Click "Signed Media Release Forms" to expand that section.
   1. This is where you can view any Media Release Forms associated with the project, or you can upload one. 
2. Click "Add a signed release form".
3. Enter the minor's first name. 
4. Enter the minor's last name. 
5. Click "Choose File". 
6. Navigate to where you saved the signed Media Release Form on your computer. 
7. Click "Upload Release Form".
